REFERENCE COPY — NOT SIGNED; NO SPACE RESERVED Version: HRS-2026-10-06-W2 Individual quote and acceptance are recorded separately before payment. Houston Rent Space — Outdoor Storage Rental Agreement Operator: HEIN-ATEN HOLDINGS LLC, doing business as Houston Rent Space. Facility and notice address: 2940 FM 528 Rd, Suite B, Webster, TX 77598. Contact: rent@houstonrentspace.com or the website rental account. Base rates: 10 × 25 feet — **$65/month**; 10 × 30 feet — **$85/month**; 12 × 50 feet — **$125/month**. Applicable tax is separate and must be shown before payment. Dimensions are approximate; Operator must confirm actual clearance and suitability during the review after payment and before approving the rental or authorizing move-in. **Fees and security deposit:** security deposit equal to one month of base rent ($65, $85 or $125 for the selected size); setup fee $0; late fee $10 once per unpaid monthly installment, after five full calendar days past the due date; returned-payment fee $30 for a qualifying returned payment, not automatically for every declined card. The security deposit may be applied to unpaid rent, applicable taxes, lawful fees, and documented damage or cleanup caused by Tenant. Operator will provide an itemized accounting of deductions and initiate refund of the remaining balance within 10 business days after the rental ends and the space is cleared. Bank posting time may be additional. The deposit does not automatically replace the final month's rent. No daily late fees, interest, or automatic collection surcharges. Any later fee change requires the notice and acceptance process in Section 12. ## 1. Space and permitted use Operator rents the identified bay to Tenant for the exclusive storage of the approved property. Tenant controls, parks, locks, and cares for that property. Common driving lanes and entrance areas are shared. This agreement does not provide valet service, vehicle maintenance, a warehouse receipt, or custody of keys. Operator will not ordinarily drive or handle the stored property. Each space is limited to one vehicle or stored item, specifically described in the rental agreement. A boat sitting on its transport trailer counts as one combined stored item, provided both are identified in the rental agreement and the entire combination fits within the space. No additional vehicles or items may occupy the space. Tenant must obtain Operator approval and update the property description before substituting a different vehicle or item. Everything, including hitches, ladders and accessories, must fit inside the approved boundaries without obstructing lanes. Tenant may not sublet, assign or transfer the space or rental agreement to another person. A replacement renter requires Operator approval and a new rental agreement. Approval of an additional driver does not transfer the rental or authorize storage of property for undisclosed third parties. ## 2. Term, rent, and monthly renewal Tenant must submit the rental information, accept the agreement and complete the required initial payment before Operator begins reviewing the rental request. Initial payment includes one full month of rent, the separate one-month security deposit and applicable tax. The request remains pending Operator approval after payment. Operator will review the paid request and communicate approval, rejection or any missing information needed to complete the review within 24 hours after successful initial payment, including weekends. This review timeframe is not a guarantee of approval or immediate gate access. Each new rental requires Operator approval before Tenant receives gate access or permission to move in. Operator will review the assigned space, identified stored vehicle or item, accepted agreement, completed payment and required insurance attestation or supplied insurance information before approving the rental. The paid rental period begins on the approved start date when the assigned space and authorized gate access are available to Tenant, after these requirements and approval are complete. Time awaiting Operator review before that date does not consume the prepaid rental month. That approved start date establishes the monthly billing anniversary; the payment-submission date does not. If Tenant chooses to arrive later, the rental start date and billing anniversary do not change. A quote, successful payment, or payment-page redirect alone is not permission to enter or move in. Gate access may be activated only after approval. **THIS IS A MONTH-TO-MONTH RENTAL THAT AUTOMATICALLY RENEWS UNTIL ENDED UNDER SECTION 4. RENT IS DUE IN ADVANCE ON THE MONTHLY BILLING DATE SHOWN ABOVE. THERE IS NO FIXED SIX- OR TWELVE-MONTH COMMITMENT.** If that billing day does not occur in a month, that month's payment is due on its last day; the original billing day applies again when available. The monthly billing date is the anniversary of the agreed rental start date. Before Operator review, Tenant pays one full month of base rent in advance plus the separate one-month security deposit and any applicable tax. This prepaid month begins on the approved start date described above. Starting late in a calendar month does not reduce the initial rent to the remaining days of that calendar month. For example, a rental beginning October 25 has its first rental period from October 25 through November 24, with the next rent payment due November 25. Operator will identify each billed period on the invoice. Payment applies first to unpaid base rent, then applicable tax, then separately itemized lawful fees. A failed payment does not constitute payment. Tenant should promptly report billing errors; nothing here shortens statutory dispute rights. Operator will correct duplicate or erroneous charges. ## 3. Automatic payments — separate authorization Automatic monthly payments are required for online rentals. At signup, Tenant must provide a payment method and separately authorize the payment processor to charge recurring rent and applicable tax on the agreed monthly billing anniversary. The security deposit is collected separately as a one-time initial charge, not a recurring monthly charge. Operator must disclose the amount, frequency, first charge date and cancellation method before obtaining payment information and provide a copy of the authorization. This rental agreement alone does not authorize unspecified withdrawals. Tenant may revoke automatic-payment authorization by emailing rent@houstonrentspace.com or using the website account control. Operator will also honor any other method required by applicable law. This does not limit Tenant's separate right to contact the financial institution to stop a covered payment. Operator will promptly stop future automatic charges in accordance with the authorization and applicable law. Revoking payment authorization does not itself erase charges already owed or complete a move-out. Any remaining lawful balance will be invoiced separately. Damage claims and collection expenses are not automatically debited under a general rent authorization. For covered consumer bank-account transfers, Operator will use a signed or similarly authenticated authorization, furnish a copy, and give legally required advance notice of varying transfers. Tenant retains applicable bank stop-payment and dispute rights. ## 4. Cancellation, move-out, and refunds Before Operator approves a paid rental request, Tenant may withdraw the pending request by email to rent@houstonrentspace.com or through the website and receive a full refund of the initial payment, including prepaid rent, security deposit and any tax collected, without setup or processing-fee deductions. The 30-day rental cancellation notice period does not apply to withdrawal received before approval. Such a withdrawal prevents the request from being approved or used to activate gate access. Operator will initiate the full refund within 10 business days after receiving the withdrawal. The bank or payment provider may require additional posting time. The automatic full-refund option for customer withdrawal ends when Operator approves the request. After approval, customer cancellation is governed by the 30-day notice and final-rent proration provisions below, even if the agreed rental start date has not yet arrived, unless Operator approves an earlier release. This cutoff does not remove refunds owed for Operator rejection or inability to provide the agreed space, or rights required by law. **EITHER PARTY MUST GIVE 30 CALENDAR DAYS' NOTICE TO END AN ORDINARY RENTAL. NONPAYMENT, OTHER BREACHES AND EMERGENCIES ARE ADDRESSED SEPARATELY BELOW, SUBJECT TO APPLICABLE LAW. TENANT MUST GIVE CANCELLATION NOTICE BY EMAIL TO RENT@HOUSTONRENTSPACE.COM OR THROUGH THE WEBSITE CANCELLATION REQUEST. PHONE CALLS ARE NOT ACCEPTED AS CANCELLATION NOTICE, EXCEPT WHERE APPLICABLE LAW REQUIRES OTHERWISE. NO RETENTION CALL IS REQUIRED.** The cancellation request must include the intended move-out date. Operator will review that requested date against the 30-day notice requirement and send written confirmation of the agreed rental end date and final balance. A requested date alone does not waive the notice requirement. A delay in confirmation does not postpone a valid notice received by email or through the website. The ordinary termination date is 30 calendar days after receipt of notice, or a later date Tenant requests. Operator may agree in writing to an earlier end date. Tenant must remove all property, leave the bay reasonably clean, return any issued physical access device, and notify Operator by email to rent@houstonrentspace.com or through the website when removal is complete and the space is cleared. This move-out confirmation is separate from the advance cancellation notice and does not by itself change the agreed rental end date. A move-out report initiates Operator review; it does not automatically release the space for another renter or process a security-deposit refund. Operator must verify that the space is cleared before either action. Operator will promptly verify and close the rental; an inspection delay alone will not extend rent when timely removal can be established or postpone the agreed refund-initiation deadline. Rent remains due through the termination date. A final partial billing period is prorated using the base monthly rent divided by the actual number of days in that billing period, multiplied by the days owed, with tax adjusted as required. Removing property before the agreed rental end date does not shorten the notice period or the rent obligation. Rent remains due through that agreed end date unless Operator approves an earlier release in writing. Operator will provide the final accounting and initiate any refund within 10 business days after the later of the termination date or actual move-out. A disputed damage claim will not delay refund of an undisputed amount. Operator will initiate a refund of a confirmed duplicate or erroneous charge within 10 business days after confirming the error. The payment provider may require additional posting time. Lawful, itemized amounts owed may be deducted with an accounting. If Operator declines a paid rental request before move-in, Operator will initiate a refund of the entire initial payment, including prepaid rent, the security deposit and any tax collected, within 10 business days after declining the request. No setup fee or processing-fee deduction will be withheld from that refund. The bank or payment provider may require additional posting time. If Operator cannot provide the agreed space at commencement, Tenant may decline a substitute and receive a full refund, which Operator will initiate within 10 business days after Tenant declines. If Operator ends availability sooner than its required notice for a reason not caused by Tenant, unused prepaid rent will be prorated and refunded. Statutory refund and termination rights control over inconsistent terms. Property remaining after termination does not become Operator's property. Tenant remains responsible for lawful charges for continued occupancy at the existing daily prorated rate and actual damages caused by the failure to vacate; there is no automatic penalty multiplier. Operator must use lawful remedies to recover possession or dispose of property. ## 5. Late payment and other breaches The disclosed $10 late fee may be assessed once for a monthly installment remaining unpaid after five full calendar days following its due date. No additional late fee is charged on that installment in later months. Operator may waive fees and offer a written payment arrangement. For an ordinary nonmonetary breach, Operator may require correction within a deadline appropriate to the nature and urgency of the violation. No fixed contractual grace period is promised. Operator will comply with any applicable legal requirements for notice, an opportunity to correct, and waiting periods. Immediate protective action may be appropriate for an actual emergency, unlawful activity, or an imminent safety threat, subject to law. This provision does not authorize automatic towing, sale, disposal, or gate lockout. A contract termination notice does not substitute for a statutory lien, towing, or court notice. No automatic vehicle sale, disposal, or gate lockout is authorized merely because a payment processor reports a failed charge. Operator must verify the account, applicable notices and protections, and a lawful enforcement method before restricting access as a collection remedy. ## 6. Access and site rules The normal tenant access schedule is 24 hours a day, seven days a week, **only after Operator confirms that access has been activated**. Emergency conditions, government orders, repairs, or a safety incident may temporarily limit access. Operator will give reasonable advance notice when practicable and provide reasonable assistance with retrieval. This is not a promise of continuous staffing, emergency roadside service, or uninterrupted equipment operation. Tenant may designate additional named drivers, subject to Operator approval and recording in the rental agreement or an approved update. An approved driver may access and retrieve the listed stored property within the authority granted by Tenant. Each approved driver must use individually assigned access credentials; approval does not permit credential sharing or storage of additional property. Use only your assigned access credentials and approved entrances. Do not share gate credentials, allow unauthorized vehicles to follow you through the gate, enter by following another vehicle without your own authorization, prop the gate open, or obstruct another tenant. The facility speed limit is 5 mph. Drive more slowly when conditions require. Accompany visitors and supervise children. Operator may require reasonable identity and authority verification before allowing a person to remove property. THE SPACE IS FOR STORAGE ONLY. No living, sleeping, camping, lodging, residential use, repairs, maintenance work, or business activity is permitted in a stored vehicle or in the rented space. No washing, fueling, dumping, sewage discharge, or utility hookups are permitted. Tenant may access the space to place, secure or remove the approved stored property; this access does not authorize other activities or use of the space as a worksite. Do not store illegal or stolen property, waste, perishables, live animals, explosives, loose fuel containers, or hazardous materials. Ordinary fuel in the approved item's designed fuel tank and properly secured manufacturer-installed systems are permitted only to the extent lawful and safe. No leaking fuel, oil, sewage, or other fluids. Report leaks immediately and arrange lawful cleanup. ## 7. Property identification and authority Tenant represents that Tenant owns the property or has the owner's written authority to store and remove it. Provide accurate identification for every vehicle, trailer, boat, outboard motor or equipment item, including its owner and known secured lenders. Operator may request reasonable proof. Do not surrender original titles or sign blank title-transfer documents as a condition of storage. Stored vehicles and trailers must remain capable of being driven or safely towed out of the facility. Tenant is responsible for maintaining that ability and arranging removal when required under this agreement. This requirement does not permit repairs or maintenance work on-site and does not authorize Operator to tow or move property without lawful authority. Update contact, ownership, lienholder, insurance and property details promptly, and before substituting an item. Vehicles operated on public roads must meet applicable legal requirements. Expired registration alone is not an automatic authorization to tow. The property schedule must identify the item’s type, year, make/model, color, VIN/HIN or other serial number, registration and state, owner and address, known lienholders and addresses, overall dimensions and estimated value. Submit property and insurance information after payment for owner review; contact rent@houstonrentspace.com to supply any additional schedule information. These details must be completed before move-in. Use an additional identification schedule only for components of the same approved stored item, such as a boat and its transport trailer; it does not authorize additional vehicles or items in the space. Declared value is information for administration; it is not a $5,000 liability cap or a representation that expensive vehicles are worth less than their actual value. ## 8. Outdoor exposure, insurance, and responsibility This is uncovered outdoor storage. Property can be exposed to rain, heat, wind, hail, hurricanes, flooding, pests, theft and vandalism. No representation is made that the facility is flood-free or that gates, lighting or cameras prevent loss. Operator does not sell or provide property insurance under this agreement. Security cameras are installed and recording at the facility, but Operator does not guarantee continuous operation, coverage of every area or incident, or that recordings will be retained, recoverable or available. Recordings are not a service promised to Tenant, and footage is not guaranteed to be provided to customers. Operator will comply with applicable legal obligations concerning recordings. **TENANT MUST MAINTAIN INSURANCE APPROPRIATE TO THE STORED PROPERTY AND ITS VALUE, INCLUDING PHYSICAL-DAMAGE COVERAGE THAT PERMITS OFF-PREMISES OUTDOOR STORAGE, AND ANY LEGALLY REQUIRED VEHICLE LIABILITY COVERAGE. TENANT SHOULD CONFIRM STORM, FLOOD, THEFT AND STORAGE EXCLUSIONS WITH TENANT'S INSURER. OPERATOR'S PROPERTY INSURANCE IS NOT A SUBSTITUTE.** Before move-in, Tenant must affirmatively attest that the required insurance is in force or provide insurer and policy information or insurance documentation. Tenant agrees to maintain the required coverage throughout storage. Tenant is responsible for confirming with the insurer that coverage applies to the identified property at this outdoor storage location and for notifying Operator promptly of a lapse or cancellation. Tenant may provide insurer and policy information or insurance documentation instead; routine upload of a policy or insurance certificate is not required unless applicable law or a binding insurance requirement requires it. Accepting an attestation or policy information is not verification or a guarantee of coverage by Operator. This obligation does not release Operator from liability imposed by law or replace Operator's own insurance. If suitable coverage is unavailable, Tenant must contact Operator before move-in; no exception is automatic. The separate property-and-insurance step records the required insurance attestation or supplied policy information. Accepting this agreement alone is not an insurance attestation. Tenant is responsible for securing the stored property, making hurricane and other severe-weather preparations, and arranging any evacuation or removal of the property. Houston Rent Space does not provide a vehicle-moving or evacuation service. Tenant should plan ahead, act only when safe, and obey public safety orders; this agreement never requires unsafe travel. This provision does not release Operator from duties or liability imposed by law. Each party remains responsible to the extent imposed by law for its own acts or omissions. Tenant is responsible for reasonable, documented damage or cleanup costs caused by Tenant's breach or negligent or wrongful conduct, including that of Tenant's authorized visitors where legally attributable to Tenant. No clause releases Operator from its own negligence, gross negligence, intentional misconduct, or nonwaivable duties. No blanket indemnity or waiver of consumer rights is created. ## 9. Limited entry and emergencies Operator may inspect the exterior of the bay and enforce reasonable site rules. Operator will request Tenant's cooperation for routine access to locked property. Entry into, movement of, or removal of property without Tenant's contemporaneous permission requires lawful authority or a genuine emergency justifying that action. When lawful and practicable, Operator will document the reason, condition of the property, contractor used and notice to Tenant. This section is not consent to repossession for unpaid rent. ## 10. Contractual lien and lawful enforcement **CONTRACTUAL LANDLORD'S LIEN: TO THE EXTENT TEXAS PROPERTY CODE CHAPTER 59 APPLIES, TENANT GRANTS OPERATOR A CONTRACTUAL LANDLORD'S LIEN ON PROPERTY STORED IN THE BAY TO SECURE LAWFUL DUE AND UNPAID RENTAL CHARGES. AFTER DEFAULT, OPERATOR MAY SEIZE AND SELL PROPERTY ONLY AS PERMITTED BY THAT CHAPTER, INCLUDING EVERY REQUIRED NOTICE AND WAITING PERIOD. FOR PROPERTY ELIGIBLE UNDER SECTION 59.051, TENANT ALSO AUTHORIZES TRANSFER AND TOWING TO A LAWFUL VEHICLE STORAGE FACILITY UNDER SECTION 59.052 ONLY AFTER ALL STATUTORY CONDITIONS ARE MET. THIS DOES NOT AUTHORIZE AN IMMEDIATE TOW OR TRANSFER OWNERSHIP TO OPERATOR. REDEMPTION, OWNER/LIENHOLDER NOTICE, MILITARY-SERVICE AND OTHER NONWAIVABLE RIGHTS REMAIN IN FORCE.** The rental-agreement consent includes acknowledgment of this conspicuous provision. ## 11. Notices and military service Routine communications may use the contact information supplied by Tenant. Statutory notices must use the delivery method and content required by the controlling law; a text message, routine invoice, or ordinary reminder is not a substitute. **IF TENANT ELECTS TO PROVIDE AN EMAIL ADDRESS FOR LEGAL NOTICES BELOW, OPERATOR MAY GIVE NOTICES OF CLAIM UNDER TEXAS PROPERTY CODE SECTION 59.043 BY EMAIL TO THAT ADDRESS, AS THE LAW PERMITS. TENANT MAY INSTEAD CHOOSE POSTAL NOTICE.** This election does not replace different notice requirements for titled owners or lienholders. Legal-notice delivery is a separate election, to be recorded with the completed tenant contact schedule before move-in. Neither the agreement checkbox nor electronic-record consent elects email delivery of statutory lien notices. Tenant may choose email or postal delivery and update that election or address by written notice. Changes apply prospectively after receipt; Operator will confirm the change. **PLEASE NOTIFY OPERATOR PROMPTLY IF YOU ARE IN MILITARY SERVICE OR BECOME ENTITLED TO MILITARY-SERVICE PROTECTIONS. FAILURE TO VOLUNTEER THAT INFORMATION DOES NOT WAIVE THOSE RIGHTS.** Operator will honor the Servicemembers Civil Relief Act and applicable Texas protections and will not enforce a storage lien where a prior court order is required without obtaining that order. Tenant may supply military-service information separately to rent@houstonrentspace.com. ## 12. Changes, privacy, and governing law Operator will give at least 30 calendar days of advance written notice before a base-rent increase. The new rate takes effect on a monthly billing anniversary after the notice period has elapsed. Operator may change reasonable ordinary site rules with immediate effect, subject to applicable law and this agreement. No fixed advance-notice period or written-notice requirement is promised for ordinary site-rule changes unless required by law or this agreement. Current rules will be made available at the facility or through tenant communications. This provision does not authorize retroactive penalties or override the separate provisions for rent increases, charges, termination, or material contract changes. Tenant may terminate before a rent increase takes effect without being forced into an additional period at the increased price. Material changes to liability, dispute rights, lien rights or payment authorization require affirmative agreement; posting a revised webpage alone does not amend a signed agreement. Legally required tax changes will be itemized and communicated, with any required payment notice or consent. Operator will use rental records, property identifiers, payment references and access logs to administer storage, safety, billing and lawful claims, subject to the published privacy notice and applicable law. The privacy notice must be provided before collecting customer information. This agreement does not consent to advertising texts or sale of personal information. Do not send full card details or account credentials to Operator by email. Texas law governs, subject to controlling federal law. Either party may bring a dispute in a court with proper jurisdiction and venue; small-claims and nonwaivable consumer rights are preserved. There is no mandatory arbitration, jury waiver or class-action waiver in this agreement. Invalid provisions are severable to the extent lawful. This schedule, agreement, property schedules and expressly signed addenda constitute the agreement; an unapproved website statement does not alter it. ## 13. Electronic signing and copies The size, stored-item category and itemized quote displayed with this agreement form the initial rental schedule. Your entered full name, affirmative agreement consent and recorded date/time are your electronic signature. Payment authorization and electronic-record consent are separate. Signing does not charge you or authorize move-in. After payment, the property and insurance schedule is submitted for owner review. Owner approval must identify the assigned bay and available start date, which establishes the billing anniversary; these are not represented as already approved when you sign. Complete the tenant contact and property information before move-in. Any changed price or material term requires a new affirmative agreement rather than an overwrite of this record. You may download the unsigned quote before signing and your accepted agreement, quote, separate payment authorization and electronic-record disclosure afterward. The signed copy includes the document version, name and acceptance time. Later website updates do not replace your saved terms. Keep the approval and completed schedules with that copy. Tax and stored-property definitions RV or motorhome: A motorhome, fifth wheel, travel trailer or pop-up camper. Storage rent is taxable. Boat: A boat, alone or on its boat trailer. An empty trailer belongs in Trailer. Boat storage rent is not taxable. Trailer: A trailer stored by itself, including a utility, freight or empty boat trailer. Storage rent is taxable. Work vehicle: A road-going truck, van, pickup or other motor vehicle. Storage rent is taxable. Equipment: Non-road equipment only, such as standalone heavy equipment or a golf cart. This excludes road vehicles, trailers and equipment permanently mounted on a road trailer. Qualifying equipment storage rent is not taxable. If unsure, email rent@houstonrentspace.com before choosing this category. The quoted sales tax applies to rent for the selected category. The refundable one-month security deposit is separate from rent and has no sales tax at collection. If the deposit is later applied to a taxable charge, the itemized accounting will include applicable tax. Prepaid first-month rent is not a security deposit. Tax rates apply for the dates documented in the saved quote; a later rate change is subject to the agreement’s notice and payment-authorization terms. SEPARATE AUTOMATIC-PAYMENT AUTHORIZATION I authorize HEIN-ATEN HOLDINGS LLC, doing business as Houston Rent Space, through its payment processor, to save the payment method I provide and charge the initial total itemized in my saved quote when I continue through secure payment. That total includes one full prepaid month, a separate one-time security deposit equal to one month of base rent, applicable itemized tax and no setup fee. I authorize recurring monthly rent and applicable tax at the monthly total shown in my quote. The prepaid month begins on the approved available start date after owner review. The next charge is due one month after that date, and charges recur on that anniversary until the rental ends. If the day is absent from a month, payment is due on that month’s last day. The approved start date and first renewal date must be communicated before recurring charges begin. The deposit is not charged again each month. This does not authorize unspecified damage charges, collection costs or other additional withdrawals. Rent increases require at least 30 days’ advance written notice under the agreement. Legally required notices or additional consent for varying electronic transfers will be provided before charging a changed amount. I may revoke this authorization by emailing rent@houstonrentspace.com or using my website rental account; other legally required methods will also be honored. Revocation stops future automatic charges as required by law but does not cancel the rental, erase amounts owed or replace the 30-day cancellation notice. I retain applicable financial-institution stop-payment and dispute rights. I can download and retain this authorization with my signed agreement. Signing this authorization alone does not initiate a charge. ELECTRONIC RECORDS AND SIGNATURE CONSENT Scope: I choose electronic delivery and signatures for this rental’s quote, agreement, separate payment authorization, approval, invoices, receipts, ordinary rental notices and account records. Records are displayed in the website rental account or delivered to the email address I provide. This consent does not elect email delivery of statutory lien notices or marketing. Requirements: I need internet access, a current web browser and an email account. I must be able to read web text and save plain-text downloads, or print them. Before consenting, I can download the unsigned quote and terms to check that I can open and keep the record. I should not sign if I cannot access or retain it. Paper and withdrawal: I may request a paper process or paper copies, withdraw electronic-delivery consent, or update my contact information by emailing rent@houstonrentspace.com or writing to HEIN-ATEN HOLDINGS LLC, Houston Rent Space, 2940 FM 528 Rd, Suite B, Webster, TX 77598. No fee is charged for paper copies or withdrawing this consent. Withdrawal takes effect within a reasonable time after receipt, changes future delivery to paper, and does not cancel the rental, payment obligations or previously signed records. An online signup that has not been completed will require arranging a paper process. If technical requirements change in a way that materially risks my access to records, I will receive the revised requirements and an opportunity to withdraw without new fees or undisclosed conditions. By checking the separate box, I confirm I can read and save these displayed records and consent to electronic records and signatures.
